In residential care, relatives are often the only people who look in regularly from outside. Their feedback is therefore more than customer satisfaction: it is an early-warning system for units where something is tipping over. This template asks about nine aspects of care, plus the two topics where collaboration with families breaks down in practice: information when the resident's health changes, and involvement in care planning. If you run several homes, append a parameter per home and unit to the link. The dashboard then compares locations without any answer being traceable to a person.

From questionnaire to dashboard
The questionnaire produces a dashboard for home management and the provider:
- Care profile: Nine aspects as full scale distributions. You see whether a quarter of relatives are dissatisfied with "information on changes", not just a mean of 3.6.
- Homes and units compared: The crosstab of aspect against location with a minimum case count: where something is slipping the cell turns red, where too few answered it stays empty.
- Topics in the open comments: The AI-coded topics of the comments as a ranking, cross-tabulated by home and relationship to the resident.
